- Request for provision of the public service (technical inspection of self- propelled machinery) (original, 1 counterpart);
- ID of the applicant (original, 1 counterpart);
- Compulsory civil liability insurance certificate for the vehicle owner (copy, 1 copy);
- At a maximum vehicle speed of more than 20 km per hour except for equipment using caterpillar drive;
- Document confirming the applicant's right to drive the vehicle to undergo a technical examination  (original 1 counterpart);
- Power of attorney or any other document confirming the powers of the car owner's representative (for the representative of the car owner)(original, 1 counterpart);
- Car Registration Certificate (original, 1 counterpart);

For  issue of a document confirming the technical examination results     - 400.0 RUB.
